Preparing for Your Visit
New Patients — Please Bring:
- Photo ID and insurance cards
- Completed new patient forms (see above)
- Recent MRI, CT, or X-ray images and reports
- List of current medications
- Referral from your primary care doctor (if required)
For Procedures:
- Arrange a driver — you cannot drive yourself home after a procedure
- Avoid blood thinners as instructed (ask our office)
- Fast if required (your office will advise)
- Wear comfortable, loose-fitting clothing
